In which the question is asked:<\/p>\n

\u201cPaul\u2026 I\u2019m having trouble finding a reason WHY one of my patients needs to continue to attend therapy.<\/p>\n

She really needs treatment on her knee following surgery – and yet is currently resisting our suggested treatment sessions and is only attending simply because her doctor told her so. <\/p>\n

She doesn\u2019t know why she has to attend\u2026 and no matter how many questions we ask, she says \u201cn\/a\u201d to everything that we might suggest as an outcome goal we could help her achieve. <\/p>\n

She\u2019s 84, has lived on her own for 40 years and isn\u2019t all that socially active – so we\u2019re having a hard time connecting what we do to her lifestyle goals.<\/p>\n

How would you suggest we talk to this lady?\u201d.<\/p>\n

Ok\u2026<\/p>\n

Let me start by saying that I hear this type of question a LOT. <\/p>\n

Both in my own clinic, coaching my own therapists, and in my work with owners. <\/p>\n

See, problems with compliance arise more common than most PT\u2019s would dare to admit and often times it\u2019s because they get lost in their own \u201cverbiage\u201d – or, worse yet, they\u2019re still using language and asking questions they were taught in college. <\/p>\n

So lets \u201cpsycho-analyse\u201d this patient with the goal of REALLY connecting her to therapy (and getting the 8 more sessions she really needs). <\/p>\n

And here\u2019s exactly how I do it: <\/p>\n

First of all I shut my eyes. <\/p>\n

And I ask the PT to describe this patient to me in detail. <\/p>\n

I\u2019m listening\u2026.<\/p>\n

And I get told she\u2019s 84, that she\u2019s lived on her own for 40 years since her husband died and isn\u2019t socially active\u2026 and therefore isn\u2019t particularly motivated to go out of the house three times per week for PT. <\/p>\n

On the surface of it, I could see why this patient might give compliance problems. <\/p>\n

But in reality, all of these reason are THE reasons WHY she needs to come for PT. <\/p>\n

See, this lady is in a FEARFUL place (\u2026home is where she\u2019s comfortable). <\/p>\n

Possibly resents being told what to do (\u2026since her husband died, she\u2019s made ALL her own decisions). <\/p>\n

Is intent on just \u201csurviving\u201d (\u2026instead of being active day to day!)<\/p>\n

And doesn\u2019t want leave the house if she doesn\u2019t have too. <\/p>\n

So now lets talk this through:<\/p>\n

This lady has lived on her own for 40 years. <\/p>\n

So what is she clinging DESPERATELY too? <\/p>\n

\u201cIndependence\u201d. <\/p>\n

And more than that, she wants to preserve her \u201cself-worth\u201d.<\/p>\n

And the fact that she doesn\u2019t want to leave the house – that\u2019s in your favour too\u2026 <\/p>\n

\u2026because if she wants to avoid being forced to live in an assisted living place, or care home, then she needs to attend you therapy sessions which will help her to continue to live in the family home for as long as is possible. <\/p>\n

There\u2019s not a more compelling reason for an 84 year old widower to arrive for therapy on her knee, than to preserve all of these things. <\/p>\n

The problem is, no matter how many times you ask her \u201cwhat do you want from PT?\u201d\u2026 she will never tell you this simply because she\u2019s never heard of a place where you can walk in and order \u201cindependence and self worth\u201d from the menu. <\/p>\n

So how does it come up?<\/p>\n

Simples. <\/p>\n

YOU bring it up. <\/p>\n

And you tell her that you see so many other ladies in there 80\u2019s who come to see you (social proof and hope!) and what THEY all wanted to achieve was to ensure independence and mobility is intact, so that they can preserve self worth. <\/p>\n

That\u2019s it. <\/p>\n

And it works because you\u2019re connecting your treatment plan to the CORE desires of the patient who realise you GET them better than anyone else.<\/p>\n

Do that, and you\u2019ll get more compliance AND more profit at your clinic because you\u2019ll confidently and ETHICALLY be able to prescribe as many sessions as is needed, to achieve THAT goal. <\/p>\n

All right\u2026<\/p>\n

Lesson over for today. <\/p>\n

Maybe more teaching like this next time. <\/p>\n
